What is Senior Care Advantage ONE?

All Medicare patients of Buffalo Medical Group who are enrolled in a Medicare Advantage plan and have designated a Buffalo Medical Group as their primary care physician are automatically enrolled in our Senior Care Advantage ONE program.

Senior Care Advantage ONE by Buffalo Medical Group works in partnership with the 2023 Medicare Advantage plans listed below:

  • Aetna
  • Independent Health
  • Highmark
  • Humana
  • UnitedHealthcare
  • Univera Healthcare

Senior Care Advantage ONE by Buffalo Medical Group is a program built on the premise that through stronger connections, care can be transformed. Connections matter --between patients and primary care physicians; between health plans and those caring for patients; and between doctors who treat the whole person and those who specialize in a specific procedure or condition.  All of these connections play a significant role in transformative care.

Senior Care Advantage ONE is not a new Medicare Advantage plan.  Rather, it is a new approach to care delivery which focuses on preventative care, care coordination resources, when you need them, and an appreciation for the demands and constraints on your desired lifestyle. The primary care physicians of Buffalo Medical Group are committed to supporting you in reaching your health and wellness goals.

We encourage our Senior Care Advantage ONE patients to complete an annual wellness visit with their primary care physician. This comprehensive, individualized assessment provides an annual roadmap to improved health and wellness. It is also the gateway for our patients to access specialized programs designed for those who need more focused care coordination resources.

Patient Services Offered are:

  • annual Medicare Wellness Visit
  • cancer screenings and preventive care visits
  • coordinated care with other healthcare professionals and services
  • referrals to other specialists and healthcare professionals
  • patient outreach about upcoming or needed appointments
  • team support for chronic conditions

We can put you in touch with brokers that understand the Senior Care Advantage ONE program. These brokers can help you evaluate coverage options to find one that meets your needs. With your permission, we will give your contact information to a broker to contact you to discuss coverage options.
